Initiate Video or Chat Session
If you would like to initiate a Video or Chat session with me,
please follow the steps below carefully:
PLEASE READ THROUGH THE INSTRUCTIONS COMPLETELY FIRST, THEN COMPLETE THE STEPS!
1. In order for us to engage in a therapeutic relationship, you need to:
- Read, Complete and Submit the following:
- We will discuss these documents in our first session.
- If you would like me to discuss your case with your physician or other professional please read and complete the Release of Information Form.
2. Click on the Breakthrough button below. You will be directed to the website which hosts my video and chat features on a secure and encrypted platform.
3. In the upper-right hand corner of the website, click on the “Client Sign Up” link to create your account. When you have completed the form, you will be sent an email to confirm your account. Open the email and click on the link.
4. Now that you have an account, click on the badge below and you will be directed to my profile.
5. Select either a Video or Chat session on the right hand side of the page.
6. Fill in the short form. Select the medium (chat or video), date and time you wish to schedule an appointment with me.
7. Your appointment is confirmed.
8. Return to my profile and select the “Pay Now” button. Enter the value for the service you have requested and click submit. Follow the PayPal instructions.
PLEASE NOTE: ONCE PAYMENT IS MADE, I AM NOTIFIED BY EMAIL IMMEDIATELY AND I AM ABLE TO CONFIRM YOUR APPOINTMENT. I WILL NOT CONFIRM AN APPOINTMENT WITHOUT PAYMENT. PAYMENTS ARE EXPECTED BEFORE SERVICES ARE RENDERED.
